Middle School Faith Formation Registration Form

This form is for students in grades 6-8 who want to be in faith formation at Our Lady of Lourdes.

If you are in grades 6-8 and have not yet been baptized or received your First Communion, please indicate that below, so we can begin your sacramental preparation as well.

The cost of Middle School Faith Formation is $35 per student. This covers the cost of books and other materials for the classes.

Multiple Student Discount
If multiple students are enrolling from your immediate family this year, it will cost $35 to enroll the second child, and any additional children may be enrolled for free.
